Registration Renewal - 2019-2020

 

Dear Parents and/or Guardians,

 

Please find below information regarding the registration renewal of your child(children) for the 2019/2020 school year.

 

  • Access the Contrato de Prestação de Serviços Educacionais(1) (Educational Services Provisional Agreement Contract) link and after logging in on ADEWeb (Secretaria Online), click on the Documentos option. You will find, if applicable, the sibling, alumni and Educational Guarantee amendments, as described below:

  • 1.3 Educational Guarantee ammendment

  • Access the Ficha Médica(2) (Medical Health Form) link to download the file in PDF format, (except for new students who have already returned it).

  • Access the Autorização de Saída(3) (Student Pick-up Authorization Form)  link to download the file in PDF format (except for new students who have already returned it).

  • The following documents are also an integral part of the Educational Services Provision Agreement:

PLEASE NOTE:

 

  1. The above documents (1), (2) and (3) should be filled out, initialed, signed and returned to the Finance Department of OLM only, no later than 07/29/2019. A receipt protocol will then be issued.

  2. In case a written copy of the Contract(1) signed by the school representative is needed, please send us 2 (two) printed copies of the referred document. We understand that by delivering just one single copy, you agree not to receive a signed copy of the Contract(1).

  3. It is of utmost importance that the documents be returned in a single envelope, sealed and identified with the student's name and grade.

  4. Doormen, secretaries and other OLM employees, who do not work in the Finance Department, are not allowed to receive these documents.

  5. It is essential to meet the deadline above so that the system can be updated before classes start. If we do not receive the Ficha Médica(2) (Medical Health Form) and/or the Autorização de Saída(3) (Student Pick-up Authorization Form) by July 29, 2019, we will consider the information provided in last year’s forms. If documents (2) and (3) are sent after the deadline, it will take us up to 3 (three) business days to update the information in the system.

  6. As for the students who were authorized by their parents/guardians to leave school alone during the 2018/2019 school year, we clarify that they will only be released upon the return of the 2019/2020 Autorização de Saída(3) (Student Pick-up Authorization Form). No authorizations of the previous school year will be considered.

 

  • Please note that contracts with administrative pending issues haven't been sent.

  • If any other student's Contract(1) is not available, please contact Adriana or Rafael immediately at the Finance Department at (21) 2266-8275 or (21) 2266-8271.

  • The Contract(1) details are in accordance with the information provided by the person responsible for the student and registered in the school system. If there is any change, please send an email to secretary@olmrio.com.

  • Should there be any questions regarding the Educational Services Provision Agreement(1), please send an email to contrato@olmrio.com which will be temporarily active.
